Budoprutug trial data from an ongoing Phase 2 study showed an early immune response in patients with primary membranous nephropathy, Climb Bio said on October 24, 2026. The biotechnology company expects additional results in 2027.

Key Takeaways

  • Five of 13 patients receiving the 200 mg dose achieved immunologic remission at 12 weeks.
  • The 200 mg and 600 mg doses were generally well tolerated during the reported follow-up periods.
  • Enrollment was continuing in a 1,000 mg cohort.

Budoprutug trial data at 12 weeks

The PrisMN dose-escalation study is evaluating budoprutug, an experimental anti-CD19 antibody, in patients with moderate to severe primary membranous nephropathy, or pMN. Patients in the reported cohorts had previously received treatment and had high levels of protein in their urine and elevated PLA2R antibodies at baseline.

At 12 weeks, the 200 mg group had a mean PLA2R antibody reduction of 70%. Five of 13 patients, or 38%, achieved immunologic remission. Climb Bio said the findings were consistent with its earlier Phase 1b data.

Changes in PLA2R antibodies can provide an early indication of treatment activity, although complete clinical remission can take longer, said nephrologist Frank Cortazar. The early immunologic results do not establish how many patients will ultimately achieve complete remission.

Higher doses and safety follow-up

Initial data from the 600 mg cohort suggested greater drug exposure and deeper peripheral B-cell depletion than at 200 mg. Mean peripheral CD20-positive B-cell suppression at four weeks was 98% at 600 mg, compared with 74% at 200 mg.

Climb Bio said both doses had been generally well tolerated, with follow-up through week 12 for the 200 mg group and week six for the 600 mg group. Most adverse events were mild or moderate; no treatment-related serious adverse events, discontinuations due to adverse events, or deaths were reported.

Separate minimal change disease case

In a separate case study, one patient with frequently relapsing minimal change disease received 100 mg of budoprutug and experienced three years of steroid-free remission. The patient remained in remission for one year after B cells returned. Climb Bio said the observation supports further investigation in the disease; it is a finding from one patient, not the Phase 2 pMN trial.

Next milestone

Climb Bio anticipates additional PrisMN Phase 2 data in 2027 as enrollment and follow-up continue.
