Intuitive earnings for the third quarter of 2026 showed strong growth in robotic surgery activity and revenue, but a litigation charge reduced reported profit. Revenue rose 22% to $3.05 billion for the quarter ended September 30, while GAAP net income fell to $523 million from $704 million a year earlier. Excluding specified items, non-GAAP earnings per diluted share increased 25% to $3.00.

Key Takeaways

  • Worldwide procedures rose 15%, with da Vinci system placements increasing to 546 from 427.
  • A $450 million pre-tax litigation charge reduced GAAP earnings; the proposed settlement still requires court approval.
  • Management expects pricing changes to reduce da Vinci instrument revenue per procedure by 3%–5% once fully implemented.

Intuitive earnings reflect broader procedure growth

Intuitive’s third-quarter earnings release, issued October 20, 2026, showed revenue increasing from $2.51 billion a year earlier. The company attributed the growth to higher procedure volume, operating lease revenue and a larger installed base of systems.

Da Vinci procedures increased approximately 14%, while procedures using the Ion platform grew approximately 31%. Da Vinci growth was uneven geographically: U.S. procedures rose 10%, compared with 20% outside the United States.

Instruments and accessories revenue increased 18% to $1.80 billion, systems revenue rose 30% to $769 million, and service revenue grew 22% to $482 million. Recurring revenue accounted for 84% of the total, showing that the business continued to depend primarily on ongoing system use rather than equipment sales alone.

Of the 546 da Vinci systems placed during the quarter, 315 were da Vinci 5 systems. Another 291 placements were under operating leases, including 160 under usage-based arrangements. Placements therefore should not be read as equivalent to outright system purchases.

Litigation charge separates reported and adjusted profit

GAAP diluted earnings fell to $1.49 from $1.95 a year earlier. The quarter included a $450 million pre-tax charge for a proposed settlement of previously disclosed antitrust class action litigation, equivalent to $347 million after tax, or $0.99 per diluted share.

Intuitive reached the written agreement in October without admitting liability or wrongdoing. It includes a one-time payment and mutually agreed injunctive relief, but remains subject to preliminary and final court approval.

Non-GAAP net income rose to $1.05 billion from $867 million. Both GAAP and non-GAAP earnings benefited from tariff refunds worth $22 million after tax, or $0.06 per diluted share.

The non-GAAP operating margin was approximately 42%, or 41% excluding the one-time tariff refund. That distinction matters when assessing underlying profitability: the refund helped the quarter but was not recurring operating revenue.

Pricing changes will roll out through 2027

Chief Financial Officer Jamie Samath said pricing actions are expected to lower da Vinci instruments and accessories revenue per procedure by approximately 3%–5% on a constant-mix basis once fully implemented. The measures include an extended-use program, volume-based pricing and China pricing changes.

China’s charge-code price reductions are scheduled to begin in November 2026. The extended-use program is expected to start in the United States later in the first quarter of 2027, depending on regulatory clearance, followed by Europe in the second half of 2027. Samath does not expect the full revenue-per-procedure effect until the end of 2027.

Management said the measures target procedures where cost is a barrier to adoption. Samath expects a modest gross-margin effect, with cost-reduction programs offsetting it across 2027. Those are management expectations, not results already achieved.

Buybacks reduce cash as full-year targets remain in focus

Intuitive repurchased 7.8 million shares for $2.94 billion during the quarter. Cash, cash equivalents and investments ended September at $6.80 billion, down $1.83 billion during the period, as repurchases and capital spending more than offset operating cash generation.

For full-year 2026, the company expects worldwide da Vinci procedure growth of 14%–15%, closer to the midpoint. Its margin outlook assumes tariffs in effect at the time of the release remain in place through year-end; additional tariffs could materially change the outcome.

Conclusion

Procedure growth supported higher revenue and adjusted earnings, while the litigation charge drove reported profit lower. The next scheduled pricing change is in China in November 2026, followed by a conditional U.S. extended-use rollout in early 2027; the proposed settlement also awaits court approval.
