MacroGenics announced interim MGC026 trial results on October 23, 2026, reporting partial responses in six of 22 response-evaluable patients with heavily pretreated head and neck cancer. Four responses were confirmed and two remained unconfirmed. The company plans to evaluate the investigational cancer treatment with pembrolizumab in a front-line cohort beginning in early 2027.

Key Takeaways

  • The head and neck cancer cohort recorded a 27% best overall response rate, including two unconfirmed responses.
  • Severe treatment-related adverse events occurred in 48% of the cohort’s safety population, but none led to treatment discontinuation.
  • MacroGenics plans an earlier-line combination cohort and expects updated results from the ongoing study in mid-2027.

MGC026 trial findings center on heavily pretreated patients

The Phase 1 findings, presented at the European Society for Medical Oncology Congress in Madrid, included an expansion cohort for recurrent or metastatic squamous cell carcinoma of the head and neck, known as SCCHN. Patients evaluable for response had received a median of three prior systemic treatment lines.

Six of 22 patients had partial responses, producing the reported 27% best overall response rate. Disease control was recorded in 14 of 22 patients, or 64%. Five responses occurred among the 16 patients with HPV-positive disease.

These are preliminary findings from a small, open-label study. The inclusion of two unconfirmed responses matters: the reported 27% figure is not a confirmed response rate and does not establish effectiveness in patients receiving their first treatment.

Separately, in dose escalation across 10 tumor types, five of 17 response-evaluable patients receiving at least 6 mg/kg had confirmed responses. That group had a 29% confirmed objective response rate and a median response duration of 10.2 months. Those results should not be confused with the head and neck expansion cohort.

Safety results show substantial blood-related toxicity

Across all 85 treated patients, Grade 3 or higher treatment-related adverse events occurred in 51%, and treatment-related events led to discontinuation in 5%. No Grade 5 treatment-related adverse events were reported. The most common severe events were neutropenia, at 27%, and anemia, at 25%.

In the head and neck cohort, Grade 3 or higher treatment-related events occurred in 48%. Anemia affected 30% and neutropenia 26%. No treatment-related events led to discontinuation in that cohort.

No cases of interstitial lung disease, pneumonitis or ocular toxicity were observed in the reported safety populations. MacroGenics described the safety profile as manageable, but the early-stage sample does not establish the absence of these risks.

What the investigational treatment targets

MacroGenics’ MGC026 product overview describes an antibody-drug conjugate that combines a B7-H3-targeting antibody with a topoisomerase I inhibitor-based drug payload. This provides background on the treatment’s design, rather than verification of the newly announced trial results.

The ongoing Phase 1 study evaluates safety, tolerability, drug exposure and preliminary antitumor activity in advanced solid tumors. MGC026 remains investigational; these findings are a clinical-development update, not a regulatory approval.

Earlier-line combination testing is the next planned step

MacroGenics plans to initiate an expansion cohort in early 2027 combining MGC026 with pembrolizumab for front-line treatment of recurrent or metastatic SCCHN. Eligible patients would have PD-L1-positive disease, defined as a combined positive score of at least 1.

The proposed cohort would test a different treatment setting from the heavily pretreated population described in the interim results. Enrollment also continues in Stage 2 of the existing head and neck cohort, with updated data expected in mid-2027.

Conclusion

The MGC026 trial has produced early response signals alongside substantial severe toxicity. The next planned milestones are the early-2027 combination cohort launch and the mid-2027 update from the ongoing head and neck study.
